New Client


For your convenience, you may now submit intake paperwork electronically through our HIPAA compliant software or you may print the forms, fill them out and bring in with you to your first appointment.  All forms must be fully completed prior to your first session. A $100 reservation fee will be charged at the time of scheduling an appointment. This will be refunded if you cancel your appointment with 24 hours or more notice.

This form contains your rights as a client, in regards to your protected health information.

This allows Healing Grace and/or your therapist to release protected information or talk to another person and/or organization about your care. Please note: Psychotherapy notes will not be released without a formal court ordered subpoena.

This document clearly states our cancellation policy and what options you have to avoid a late cancellation fee.

Please note: All parents/guardian's of a child client MUST read and sign the above agreement before the first session.
