New Client


For all new client appointments, please call our office at (816) 2416-4465 ext. 1 for Scheduling.

Our Scheduling Team will then guide you through our process for scheduling an intake appointment. A $100 reservation fee will be charged at the time of scheduling an appointment. Once your appointment is scheduled and the reservation fee is paid, we will then send you a confirmation email as well as a link to sign in to our portal to complete your paperwork electronically through our HIPAA compliant software.

If you would rather complete your documents a different way, please let our scheduling team know when making your first appointment.

All forms must be fully completed prior to your first session.

If you need to cancel or reschedule your appointments, please do so no later than 24 hours before your appointment to avoid a cancellation charge.

This form contains your rights as a client, in regards to your protected health information.

This allows Healing Grace and/or your therapist to release protected information or talk to another person and/or organization about your care. Please note: Psychotherapy notes will not be released without a formal court ordered subpoena.

This document clearly states our cancellation policy and what options you have to avoid a late cancellation fee.

Please note: All parents/guardian's of a child client MUST read and sign the agreement on our portal before the first session. You may preview the agreement by clicking on the above link. Any documentation showing joint custody or custody arrangements must also be provided before or at your first session.
